To pay in-person: We always gladly accept in-person payments at our office.
To pay by phone: for services provided at any time, call the Southwest office at 503-292-5882.
To pay online for services provided in 2023 or before, you can find the old portal to pay your bill HERE , or using the QR code below. You will require info contained within your statement to make a payment including the account number, date of birth and zip code of the patient in question.
